– Contact details, civil status and home address can be changed online in ORUS, free. Name and birth date are not on that online list, so take them to your RDO.
– Civil status on paper: Form 1905 in two originals with the marriage certificate or court decision.
– BIR does not publish a separate list for a wrong name or birth date. Ask your RDO what proof it needs before you go.
– Moving to another RDO, with no business: Form 1905 in two originals.
