SSS Sickness Benefit: Checklist
Name: [Your name]. Sick from [First day] to [Last day].
1. What you need
☐ At least 3 monthly contributions in the 12 months before the semester of your sickness. The semester is two quarters ending with the quarter you fell ill. Sick in November 2026? Count July 2025 to June 2026.
☐ More than 3 days of sickness, in hospital or elsewhere with SSS approval.
☐ Employee: your paid sick leave used up. SSS pay starts after it.
☐ Medical certificate and copies of your medical records.
☐ For online filing: a My.SSS account and an approved bank account.
2. The form
– SSS lists a Sickness Notification, a Reimbursement form, a Benefit Application for those without an employer, and a Medical Certificate. Check the current form in My.SSS or at a branch.
3. Your application
– Employee: tell your employer within 5 calendar days from the first day.
– Unemployed or self-employed: tell SSS directly, also within 5 calendar days.
– No notice is needed if you were hospitalized. Employees also need none if it happened at work.
– Late notice: the sickness counts from no earlier than 5 days before you gave it.
4. Where to file
– Employee: your employer pays you, then gets it back from SSS. It must notify SSS within 5 calendar days of your notice.
– Unemployed, self-employed or OFW: you claim from SSS. SSS pages point to My.SSS.
5. Time limits
– Claim within 1 year: SSS pays only for sickness in the year before it receives the claim. Hospital stay: within 1 year from the last day.
– Up to 120 days a year, no carry-over, and 240 days for one confinement.
– Amount: 90% of your average daily salary credit, from the first day. Average: your 6 highest monthly credits in those 12 months, divided by 180.
– Employer pays on each payday, or on the 15th and the last day of the month.
6. Keep it
– Keep your notice, medical papers and pay slips.
– If you told your employer in time and the employer was late, the employer cannot take the money back from you.
