Subject: Maternity notification and maternity leave application (R.A. 11210)
Dear [HR MANAGER],
I inform you that I am pregnant. My expected date of delivery is [EXPECTED DATE OF DELIVERY]. Attached is my Maternity Notification Form with proof of pregnancy ([PREGNANCY TEST SIGNED BY THE PHYSICIAN / ULTRASOUND]), for transmittal to SSS through the employer's My.SSS account.
I apply for maternity leave of [105 / 120 / 60] days with full pay under Republic Act No. 11210 (Expanded Maternity Leave Law), to start on [START DATE]. At least 60 days of the leave will be after the delivery.
Under the SSS rules, the full payment of the maternity benefit is advanced by the employer within thirty (30) days from the filing of the maternity leave application. Based on my contributions, the SSS maternity benefit is estimated at ₱[AMOUNT] — the six highest monthly salary credits in the 12 months before the semester of delivery ÷ 180 × [NUMBER] days. SSS computes the final amount. I also ask about the salary differential up to my full pay, unless the company is exempted.
