Dear [NAME],
Request for a written statement of reasons for my dismissal — section 92, Employment Rights Act 1996
I was employed by you as [JOB TITLE] from [START DATE]. My employment was ended by you:
( ) with notice given on [DATE OF NOTICE], ending on [END DATE]
( ) without notice, on [DATE OF DISMISSAL]
( ) when my fixed-term contract ended on [DATE] and was not renewed
Employee or payroll number: [NUMBER].
I was not given the reasons in writing. Under section 92 of the Employment Rights Act 1996, I request a written statement giving particulars of the reasons for my dismissal.
On the effective date of termination I had been, or will have been, continuously employed for [YEARS AND MONTHS], which meets the qualifying period in section 92(3). [IF RELEVANT: Under section 92(7), the effective date of termination is the date the statutory minimum notice would have ended, [DATE].]
Section 92(2) requires the statement to be provided within 14 days of this request, that is by [DATE = DAY YOU RECEIVED THIS + 14 DAYS]. Please send it to [EMAIL OR POSTAL ADDRESS].
Please note that, under section 93, an employment tribunal may hear a complaint if an employer unreasonably fails to provide the statement, or if the reasons given are inadequate or untrue, and must then award two weeks' pay.
This request does not affect any other rights or claims I may have.
Yours sincerely,
