Dear [NAME],

Request for form P45 — regulation 36, Income Tax (PAYE) Regulations 2003

I was employed by you as [JOB TITLE] from [START DATE] until [LAST DAY OF EMPLOYMENT]. My employee or payroll number was [NUMBER].

I have not received my form P45. Under regulation 36 of the Income Tax (PAYE) Regulations 2003, when an employment ends the employer must complete form P45 and provide Parts 1A, 2 and 3 to the employee on the day the employment ends or, if that is not practicable, without unreasonable delay. My employment ended on [LAST DAY OF EMPLOYMENT], so the P45 is now overdue.

My new employer needs Parts 2 and 3 to work out my tax correctly.

Please:

send me Parts 1A, 2 and 3 of my P45 by [DATE] — by email to [EMAIL] or by post to the address above;

make sure it shows the leaving date [LAST DAY OF EMPLOYMENT], my tax code, and my total pay and tax from 6 April to my leaving date;

confirm that my final pay of £[AMOUNT] paid on [DATE] has been reported to HMRC.

[IF RELEVANT: You told me on (DATE) that the P45 was posted on (DATE). It has not arrived. Please confirm the address you used and the date it was sent.]

Thank you.

Yours sincerely,
