Re: T4 slip for the [YEAR] calendar year — employee [EMPLOYEE NUMBER, IF ANY]

Dear [NAME OR PAYROLL TEAM],

I worked for you as [JOB TITLE] from [START DATE] to [END DATE OR "PRESENT"]. I need my T4 slip for [YEAR] to file my income tax and benefit return, which is due by April 30, [FOLLOWING YEAR].

1. What is wrong (tick one)

( ) I have not received my T4 slip for [YEAR], on paper, by email or on your employee portal.

( ) I received a T4 slip, but it is incorrect: box [NUMBER, FOR EXAMPLE 14 / 22 / 16 / 18] shows $[AMOUNT ON SLIP], while my pay records show $[CORRECT AMOUNT]. My last pay stub for [YEAR], dated [DATE], shows year-to-date [EARNINGS / TAX DEDUCTED] of $[AMOUNT]. Copy enclosed.

2. The rules

Under subsections 205(1) and 209(1)–(2) of the Income Tax Regulations, an employer must file T4 slips by the last day of February of the following year and send each employee their copies by the same date. As I am [A FORMER EMPLOYEE / ON EXTENDED LEAVE], please send them [ON PAPER TO MY ADDRESS ABOVE / BY EMAIL TO THE ADDRESS ABOVE — I CONSENT TO ELECTRONIC DELIVERY].

If a slip you already filed is wrong, please file an amended slip with the CRA and send me the copies marked "AMENDED".

3. What I ask you to do

Please, by [DATE — 10 DAYS FROM THIS LETTER]:

send me my [YEAR] T4 slip [OR: an amended T4 slip correcting box …];

confirm whether and when you filed it with the Canada Revenue Agency, so I can check it in my CRA account;

if you believe no T4 is due, tell me why in writing.

If I do not receive the slip in time, I will report my income using my pay stubs, as the CRA allows, and attach a note giving your name and address, the type of income and the steps I have taken to get the slip.

Thank you,
