Philippine Passport (DFA): Checklist

Name: [Your name]. Applying as: [New or renewal].

1. What you need

☐ Confirmed online appointment. Only passport.gov.ph appointments are legitimate, and booking is free.

☐ New adult: PSA birth certificate on security paper, original and one copy. If it is hard to read, also the local civil registrar copy.

☐ One valid ID with one copy. National ID comes first. SSS, UMID, LTO, PRC, Postal ID and others on the DFA list also count. TIN, PhilHealth and Pag-IBIG IDs do not.

☐ Renewing an ePassport: the passport and a copy of its data page. Changed name: original PSA documents.

☐ Older non-ePassport: this counts as a new application. Bring the old passport and a copy of its data page, if you have it.

☐ Woman using her husband’s surname: PSA marriage certificate or Report of Marriage, original and copy.

2. The form

– The form is generated after you book. Courtesy Lane applicants download it from the DFA site.

– Black or blue pen. No photo needed.

3. Your application

– Book, pay at a Payment Center, then wait for the email with your confirmation packet. No email in 24 hours: call (02) 8234-3488.

– Print the packet and come in person.

– Under 18: the child comes with a parent or an authorized adult. Documents differ by case, so read the minor pages on consular.dfa.gov.ph.

4. Where to file

– A DFA Consular Office in the Philippines. Abroad: your embassy or consulate, which sets its own fees.

– Courtesy Lane without appointment: seniors, persons with disability, pregnant women, solo parents, OFWs and children 7 and under, with proof. Offices may limit the daily number.

5. Time limits

– Fee: ₱950 regular, ₱1,200 expedited, the same for new and renewal. Lost or damaged passport: ₱350 more. No penalty for an expired one.

– Regular: 10 working days in NCR, 12 elsewhere. Expedited: 5 and 7. These are estimates, the same DFA page also says 10 to 15 working days. Courier time is extra.

– Valid 10 years, 5 if you are under 18. Do not buy tickets before you hold the passport.

6. Keep it

– Check your details at release. A personal-data error means a new application and ₱950.

– Unclaimed passports are cancelled after 1 year.

– You need 6 months of validity to travel. DFA suggests renewing within a year of expiry.
