PhilHealth Registration: Checklist

Name: [Your name]. Category: [Category]. Registration has no fee.

1. What you need

☐ Filled-in, signed PMRF, and a spare copy: older pages ask for two.

☐ One copy of your birth certificate, baptismal certificate or a valid ID.

☐ Self-employed: one proof of income, such as a BIR-stamped return or a contract. No proof? State your monthly income on the PMRF.

☐ Land-based OFW: your latest OEC, job order or contract, a copy of your ID and income proof.

☐ Dependents: marriage or birth certificates that show the relationship.

☐ Through a representative: an authorization letter and one government ID from each of you.

2. The form

– PMRF, UHC version of January 2020, from the PhilHealth site or any Local Health Insurance Office (LHIO).

– Capital letters, N/A where it does not apply. Write your mother’s name as on your birth certificate.

– TIN and PhilSys number are optional. A mobile number is required, and an email for OFWs. Foreign nationals use PMRF-FN.

3. Your application

– Employee: give the PMRF to HR. Your employer adds you to its ER2 report and pays in. Already have a PIN? Give it to HR.

– Kasambahay: KURF from you and HEUR2 from your employer.

– Sea-based OFW: through your manning agency. You need a PIN to be added to its ER2.

4. Where to file

– Self-paying: an LHIO, PhilHealth Express, an accredited partner, the Member Portal or the eGovPH app.

– OFW: any PhilHealth office or your agency, or email ofp@philhealth.gov.ph with scans and a selfie with your ID.

5. Time limits

– LHIO, up to 5 forms: your PIN and card the same visit, about 1 hour 50 minutes with the queue.

– OFW by email: 3 days, depending on volume. Portal, eGovPH and employer: no time is stated.

– Premium: 5% of monthly income, ₱500 to ₱5,000 a month. Employees pay half. Self-paying: the whole year by 31 December, with grace to 31 January.

– From 1 January 2027 self-paying members pay only with a Statement of Premium Account (SPA). Ask for it before you pay.

6. Keep it

– Every citizen is covered, and you need no card to use benefits. Unpaid premiums are still owed, with interest of up to 1.5% a month for self-paying members.

– Self-paying members can apply until 31 December 2027 to have interest for November 2019 to December 2025 waived. The premiums stay due.

– Cannot pay? Ask a DSWD or LGU social worker for an assessment.
