Request for mandatory reconsideration of the decision dated [DATE OF DECISION] — LCWRA element paid at the lower amount

Dear Sir or Madam,

I ask you to reconsider your decision dated [DATE OF DECISION]. You decided that I have limited capability for work and work-related activity (LCWRA), but you included the LCWRA element at the lower amount (£217.26 a month in 2026 to 2027) instead of the higher amount (£429.80).

I believe the higher amount should apply because (tick all that apply):

( ) I am a severe conditions criteria claimant. At least one descriptor in Schedule 7 to the Universal Credit Regulations 2013 applies to me constantly and will do so for the rest of my life (regulation 40A). The descriptor(s): [DESCRIPTOR NUMBER AND WORDING]. My condition, [DIAGNOSIS], is lifelong and was diagnosed by [CONSULTANT OR SPECIALIST, NHS TRUST OR BOARD] on [DATE OF DIAGNOSIS].

( ) I told you about my health condition before 6 April 2026. I reported it on [DATE], by [JOURNAL ENTRY / FIT NOTE / PHONE CALL / CLAIM FORM].

( ) I was already entitled to the LCWRA element before 6 April 2026 and have been entitled to it continuously since [DATE] (regulation 27A).

( ) I am nearing the end of my life. [DETAILS AND MEDICAL EVIDENCE, WITH DATES]

( ) My partner, [PARTNER'S NAME], is entitled to the higher amount, so our award should include the higher amount (regulation 27(4)).

Why the decision is wrong

[EXPLAIN IN YOUR OWN WORDS: what you cannot do, all the time, and why it will not improve. Refer to the medical evidence below. If the assessment report missed or misread something, say what.]

Evidence enclosed (not sent before; my name, date of birth and NI number are on each page):

[LETTER FROM CONSULTANT OR SPECIALIST CONFIRMING THE DIAGNOSIS AND THAT IT IS LIFELONG]

[REPORT OR CARE PLAN FROM THERAPIST, NURSE OR GP]

[OTHER EVIDENCE]

[IF LATE:] I am asking after one month because [REASON — FOR EXAMPLE, HOSPITAL STAY FROM (DATE) TO (DATE)].

Please send me the mandatory reconsideration notice in writing.

Yours faithfully,
