I respectfully request my BIR Form No. 2316 for calendar year [YEAR], covering my employment with [COMPANY NAME] from [DATE HIRED, OR 1 JANUARY [YEAR]] to [LAST DAY OF WORK, OR 31 DECEMBER [YEAR]].
Under Section 2.83.1 of Revenue Regulations No. 2-98, as amended by Revenue Regulations No. 11-2018, the employer furnishes BIR Form No. 2316 on or before January 31 of the following year — or, if employment ends before the close of the year, on the day the last payment of compensation is made.
