• Typical uses: delayed registration of birth or death, discrepancy or "one and the same person" cases, supporting documents for administrative correction of entries (R.A. No. 9048 as amended by R.A. No. 10172), and agency requests (PSA, DFA, SSS, GSIS, banks).

• Basis: Act No. 3753 (Civil Registry Law) and its implementing rules (NSO Administrative Order No. 1, s. 1993), which list among the requirements for delayed registration of birth an affidavit of two disinterested persons who know of the birth. Many Local Civil Registrars use their own form — ask first and adapt.

• "Disinterested" means not related to the subject and without interest in the outcome. Affiants should preferably be older than the subject and able to state how they know the facts.

• Notarial act: jurat (2004 Rules, Rule II, Sec. 6); each affiant must be present and identified separately (Rule II, Sec. 12).
