• DSWD Travel Clearance is required for a minor (below 18) who travels abroad alone or with a person other than his or her parent(s) or legal guardian (DSWD Citizen’s Charter, "Issuance of Travel Clearance for Minors Traveling Abroad"). Generally not required when the minor travels with a parent who exercises parental authority, but confirm special cases (illegitimate child with the father only, solo parents, guardianship, minors already residing abroad) with the DSWD Field Office.
• DSWD checklists list: application form, PSA birth certificate, parents’ PSA marriage contract (or CENOMAR/guardianship order), notarized affidavit of consent of both parents/solo parent/guardian, notarized affidavit of support with proof of the sponsor’s financial capacity, photos, the companion’s passport copy, and a waiver releasing DSWD from liability. Requirements differ for first and succeeding travel; check the current list.
• Parents abroad: sign before the Philippine Embassy/Consulate (consular acknowledgment/jurat), as DSWD checklists require, attaching passport/visa copies.
• Parental authority: Family Code, Arts. 209–211 (joint parental authority); Art. 176 as amended by R.A. No. 9255 (illegitimate child — mother). Child protection: R.A. No. 7610; anti-trafficking law R.A. No. 9208 as amended by R.A. Nos. 10364 and 11862.
• Notarial act: jurat; both affiants must appear personally.
